The Knoxville and Sevierville branches were delighted to roll out the Memory Care Program in January of 2013.
This program was developed late last year as an effort to address the unmet needs of our patients with memory loss. In East TN, approximately 22,000 individuals have some form of Alzhiemer’s disease or memory impairment. Our clinical team recognized that our patients with Alzheimer’s disease and other dementias needed specialized home health care. For example, patients often needed nurses and therapists to repeat directions or leave written notes for the patient and/or family caregivers. The staff quickly recognized the need to establish a daily routine and plan of care for each patient. Often our memory care patients had family members in need of disease education and counseling.
Our staff also learned they needed tips on communication and body language when working with memory loss patients. The branch turned to the local organization Alzheimer’s Tennessee for education and support. Alzheimer’s Tennessee, the local – nonprofit organization offered training to our staff. The training provided tips on communication, insight on disease progression and community resources. The training taught staff first hand what it is like to be a person with memory loss.
Both the Knoxville and Sevierville branches are proud to now offer our community a new way of providing home health care for a person with cognitive memory loss. In the last 3 months we have served 32 patients and families. We are working with patients that live in thier own home as well as patients that are living in assisted living centers. Our goal is delivering the right care at the right time in the right setting, allowing memory loss patients to receive specialized care. We are also working closely with our fellow Harden company The Home Option to provide families with quality private duty caregivers to meet the needs of patients with memory loss. This is an exciting new way of dellivering care and we are commited to growing this program in TN.
